This is a McCormick Care Group – COVID-19 Update for Monday, February 15, 2021.
There are no probable or suspected cases of COVID-19 among our residents or staff at this time.
On Sunday, February 14, 2021, I was advised that approved essential caregivers could begin booking appointments to receive the COVID-19 vaccination. Approved essential caregivers are those that have been attending the home regularly over the course of the pandemic. Unfortunately, other family members are not eligible for the vaccination at this time.
If you did not hear from me directly yesterday, either by email or phone, your name has not been submitted to the Middlesex-London Health Unit. Without that, you will not be able to book an appointment. If you are an approved essential caregiver, and still want to receive the vaccination, please email me at firstname.lastname@example.org or call me at 519-432-2648 Ext. 2321.
You will need to take a copy of the revised consent form with you when you go to your appointment. I included this form in the email I sent you. There will be forms available at the Agriplex, if you do not have access to a printer. Some families have advised me that they are being asked to bring proof of their being an essential caregiver at McCormick Home. Dr. Summers has confirmed you do not need to provide such proof. The proof is my submission of the list of approved essential caregivers to the health unit.
Once you have booked your first and second dose appointments, please notify me via email or phone at the above number. I have to submit this information to the ministry for statistical purposes.
