McCormick Care Group – COVID-19 Update

This is a McCormick Care Group – COVID-19 Update for Tuesday, January 05, 2021.
There are no probable or suspected cases of COVID-19 among our residents or staff at this time.
We received information today from the Middlesex-London Public Health Unit related to the Moderna vaccine. The Moderna vaccine is intended for residents and will require the administration of 2 doses, 28 days apart.
Vaccinations are voluntary, but strongly encouraged, by the health unit. While we do not have a confirmed date for our residents to receive the first round of vaccination, we have been asked to start requesting consents from residents or their substitute decision makers so the vaccine can be administered as quickly as possible.
Consent forms must be obtained by the home before the vaccine is delivered to the home. Separate consent forms will need to be completed for each dose. On our website, and included in the email version of this message, you will find the following:

Consent forms can be printed, completed, and dropped off at the home. Alternatively, you may call Arlene MacDonald, Assistant Director of Care at 519-432-2648 Ext. 2402 and provide verbal consent. If you have any questions about the vaccine or the process for consenting to the vaccine, please feel free to contact Arlene. We are asking that you notify the home of your decision no later than Monday, January 11, 2021.
